Requesting Medical Records from North Suburban Medical Center
To receive a copy of your medical record, please download our Medical Record Request form and either mail it back or fax it to our Health Information Management office. Please note that birth certificates are processed through the Colorado Department of Health Vital Statistics Department.

Depending upon request, fees may apply.
Please allow a minimum of 10 business days for processing time.
Records for physician office visits will be handled urgently to ensure continuity of care.
Birth Certificates
Certified birth certificates are processed and purchased through The Colorado Department of Public Health and Environment. North Suburban submits records of all births directly to the Department of Vital Statistics.
